Abstract

Patient-centered health care emphasizes health literacy and self-efficacy, encouraging individuals to actively engage in medical decision-making regarding their conditions.
This approach positions patients as equal partners with health professionals rather than passive recipients of care. However, in public hospitals such as Bhayangkara Hospital Semarang, patient satisfaction remains varied, especially in communication, participation, and understanding of medical information. This study aims to examine the influence of health literacy, participation in medical decision-making, and self- efficacy on outpatient satisfaction. A quantitative cross-sectional design was used involving 220 outpatients selected through purposive sampling. Data were collected using a Likertscale questionnaire adapted from the Health Literacy Questionnaire (HLQ), Patient Activation Measure (PAM-13), General Self-Efficacy Scale (GSE), and Patient
Satisfaction Questionnaire (PSQ). The data were analyzed using Partial Least Squares Structural Equation Modeling (PLS-SEM) with SmartPLS 4 to assess construct validity, reliability, and relationships among variables. The results showed that all constructs were valid and reliable, with the model explaining of the variance in patient satisfaction. Health literacy had a positive but insignificant effect on satisfaction , while participation in decision-making and self-efficacy demonstrated positive and significant effects. These findings support patient-centered care and self-determination theory, suggesting that patient satisfaction is shaped not only by medical service quality but also by patients’
active engagement and confidence in managing their health. Strengthening patient participation and self-efficacy may thus enhance satisfaction and the overall quality of healthcare services.
